Before your appointment, make sure you know these important I-693 requirements:

Complete Pages 1–3 before your appointment. Write your full legal name at the top of all 14 pages of the I-693 form, exactly as it appears on your government-issued ID.

Print the official I-693 form with the barcode visible at the bottom of every page.

Bring an original, valid photo ID, such as a passport or driver’s license.

Bring vaccination records in English, if available.
