• A hard copy of a valid government-issued photo ID (passport or driver’s license)
• A hard copy of your vaccination records (if available)
• Any prior medical records related to TB treatment (if prior history of positive Tb test results)
• Form I-693, with pages 1–3 completed (as applicable)
• Your name filled in at the designated space at the top of every page of the I-693 form

Our Mountain View clinic provides same-day appointments for I-693 exams to patients in Mountain View, Sunnyvale, Palo Alto, and surrounding Bay Area cities.
• Standard processing takes 7-10 business days.
• We also offer expedited service (3-5 business days) for an additional fee.
No, fasting is not required for the labs we perform as part of the immigration medical exam.
• That’s okay! We’ll review your history and recommend only the vaccines required by USCIS.
• We can provide any needed vaccines during your visit or help guide you to get them elsewhere.
• Yes! We offer live, HIPAA-compliant remote interpreter services in all languages.
• Interpreter support is available only during the medical exam via a secure connection between the doctor and patient.
• Absolutely! Please let us know in advance so we can schedule everyone to be seen together.
• Immigration exams are typically not covered by insurance, and payment is due at the time of service.
• We accept most major forms of payment, including HSA/FSA.
• Once your labs and results are complete, we will prepare your I-693 form and seal it in an envelope as required by USCIS.
• You’ll also receive a PDF copy for your records and will have the opportunity to review everything before we seal the envelope.
The immigration medical exam typically takes 15–20 minutes.
• Absolutely not. You’ll first meet with Dr. Rajput, who will review your history, order the required blood tests, and recommend any necessary vaccines based on USCIS guidelines.
• We’ll guide you every step of the way.
• Only one visit is needed at our clinic.
• Follow-up communication will be done via email or phone, and once everything is complete, you'll return briefly to pick up your sealed I-693 form.
