1. Government-Issued Identification
Please bring a valid, unexpired photo ID. This is required to verify your identity.
Examples include:
Passport (preferred)
Driver’s license
2. Vaccination Records
Vaccination documentation is one of the most important parts of your immigration medical exam.
Bring:
Official immunization records
Childhood vaccine history (if available)
If you do not have complete records, do not worry. We can review your history and provide required vaccines during your visit.
3. I-693 form
Please bring:
Form I-693 (pages 1-14), with pages 1–3 completed (as applicable)
Your name filled in at the designated space at the top of every page of the I-693 form
